Signal Line Protection — Framework 5 — 5M Stitch In Time

Build your own
Risk Roundtable dashboard

The Risk Roundtable is the quarterly governance mechanism that tells you whether your organization is actually hearing its signals — or burying them. Enter your own data, real or imagined, and receive a personalized dashboard with an AI-generated signal health assessment.

TEPCO held quarterly reviews. They had no mechanism that measured whether signals were being heard. This tool builds that mechanism.

How this works

Four steps to your dashboard

You will enter your organization details, four quarters of signal data, up to five signal log entries from your most recent quarter, and an assessment of your protection record. The dashboard is then generated with your data and an AI-written signal health analysis based on the 5M Signal Line Protection framework.

Step 1
Organization and period
Your organization name, reporting period, sector, and approximate size.
Step 2
Four quarters of signal data
Signal volume and average response time for each of the past four quarters.
Step 3
Signal log entries
Up to five signals raised this quarter — summary, category, status, and days open.
Step 4
Protection record
Whether signal-raisers in your organization are protected or at risk.

Use real data or invent it. The dashboard works equally well with genuine organizational figures or plausible hypothetical data. Many leaders find it useful to model a worst-case scenario first, then their actual situation.

Step 1 of 4

Organization and reporting period

This information personalizes your dashboard header and helps the AI assessment apply the right context to your signal health analysis.

If 2 out of 8 incidents this quarter had a prior signal that was not acted on, enter 0.25. If you are not tracking this yet, enter your best estimate.

Step 2 of 4

Four quarters of signal data

Enter signal volume (number of Signal Notes submitted) and average response time (days from submission to documented decision) for each quarter. The most recent quarter is your current reporting period. Use estimates if exact figures are not available.

Step 3 of 4

Signal log entries

Add up to five signals raised in your current reporting period. These appear in the signal log table on your dashboard. Include at least two for a meaningful dashboard. Use real signals from your organization or construct plausible examples.

Step 4 of 4

Protection record

The protection record is the most consequential metric in the Risk Roundtable. A single unaddressed incident of professional cost to a signal-raiser suppresses future signals across the entire organization.

No incidents — signal-raisers are demonstrably protected and rewarded
Mostly protected — no formal incidents but some informal signals of discomfort
One unresolved incident — under investigation or unacknowledged
Multiple incidents — signal-raisers have experienced clear professional cost
